A photocopy of the FRONT and BACK of your credit card.
A photocopy of your picture identification card.

China Visa Application Form:
1. Download and print out the form.
2. Complete, sign, and date the form.
3. Send the following items to our corporate office
The completed, signed, and dated Visa Application.
Original passport (s) for all passengers needing China Visas.
1 color Passport photos for each passenger applying for visas.
$160.00 fee for single entry visa (non-refundable)
4. Additional reminders:
Passport (s) must be signed.
Passport (s) must not expire within the next 6 months.
Request Visa (s) no sooner then 2 months and no later then 1 month prior to the
departure date.
